Eligibility
You are eligible for benefits following 30 days of employment if you are a regular, full-time employee working 30 hours or more per week.
You may enroll your eligible dependents in the same plans you choose for yourself, including medical, dental, vision and optional life insurance coverage. Eligible dependents may include:
- Your legal spouse;
- Your children up to end of month of age 26;
- Your unmarried dependent children over age 26 who are incapable of self-care because of a handicap and who rely on you for support
COBRA
If you are no longer employed or are otherwise losing coverage provided by CTI, you may be able to continue certain benefits under COBRA. Information about plans, how to enroll and the cost to continue benefits will be mailed to you by Benefit Resource (BRI) following the termination of coverage. You will have a minimum of 60 days to elect coverage retroactive to the date in which the coverage ended. For questions contact Benefit Resource (BRI) at 800-473-9595.
